Tsarev peak (2,389 m) is in Rila, about 7 km south-southwest of Rila Monastery. Rizvanitsa is to the northwest and Vododela to the east.
According to the Life of St Ivan of Rila by Patriarch Evtimiy, and to folk tradition, Tsar Peter I met and talked with St Ivan of Rila here. Tradition says two small hollows in the rocks below the summit are the prints of the tsar's knees.
Along the ridge between Markov vrah and the western foot of Tsarev vrah, at 2,000–2,200 m, runs a megalithic sanctuary about 5 km long: boulders cut with small basins and channels. The researcher Markov suggests it was the pagan forerunner of Rila Monastery.
A waymarked route leads to the summit from the village of Bodrost.
